Wellness and Preventive Plan

Your group also offers a Wellness and Preventive Plan that meets the individual mandate for minimum essential coverage as defined by the Affordable Care Act. If you are unsure if you elected these benefits, call Customer Service at 1-866-798-0803, Monday through Friday, 8:30 a.m. to 8:00 p.m. Eastern Time. A translation line is available for most languages.
