Complex Care
For plan members facing serious illness or injury, this program provides an RN care coordinator who acts as an advocate for the member and his or her family.
For example, the program will help patients with serious primary diagnoses, such as cancer and other significant health problems, coordinate care they receive from multiple specialists and other providers, and address personal concerns ranging from disruption of family relationships to financial worries.
Assistance includes:
- Benefit Coordination
- Information on treatment options
- Identification of resources including: financial assistance, support groups, special services, supplies, equipment, transportation, and more.
