Find a Health Care Provider

Locating a preferred provider is simple.

Step 1: Click on the name of your preferred provider network from the list below. If you’re not sure what network access you have, check your member ID card.

2: Choose a Provider Type. On the provider’s landing page, you should see a prompt to “Locate a Provider” and/or “Create a Directory” or similar wording.

Step 3: Pick a location. There are options to search for provider by: Zip code; State and County and/or City; and Provider’s Name, Facility, Specialty or Condition.

Results can be narrowed by distance from you, type of provider or specialty.

Click the prompt to create or print a personalized directory. Follow the online instructions for selecting some or all of the providers to include.

Please call the provider before making an appointment to verify that the provider continues to be part of the network.
